Your Initial Visit To Our Office


In person or via Zoom


The initial consultation is intended to acquaint us with your circumstances and acquaint you with basic information about bankruptcy.


At our meeting, you can tell us about your situation so we can assess your legal needs. We will discuss your goals and review your options. All you have to bring with you is your current driver's license and Social Security card. If you can't find your card, let us know; we will let you know what to bring as a replacement.


If bankruptcy is the right choice for you, we will provide you with a list of documentation that we will need from you, based on your particular facts, so that we can prepare your bankruptcy petition.  At the meeting, while together, we can also retrieve certain needed documentation that is available online, such as credit reports and real estate documents.


If there are alternatives to bankruptcy that may resolve your issues, we will go over them with you and help you to make an informed decision.


We cannot offer legal advice just yet; we need to evaluate your needs and circumstances to advise you as to your next steps.

Below is a list of documentation frequently requested to complete a petition:

Pay Stubs

For the period covering the 6 months prior to your visit.

Evidence of Other Income

Including tips, bonuses, commissions, child support, alimony, support from someone paying or sharing your living expenses, rental income, unemployment compensation, social security, retirement, pension, interest, dividends, or income from any other source.

If Self-Employed

A profit and loss statement for the period covering the 6 months prior to your visit and/or copies of monthly statements for any business bank account for the 6-month period prior to your visit.


Income Tax Returns 

Copies of your last two filed tax returns.

Bank / Financial Statements

Monthly statements from any bank, credit union, or financial account on which your name appears, regardless of balance, for the 6-month period prior to your visit.

Retirement Accounts

Your most recent statement from any retirement account(s) in your name, including 401(k) accounts, IRAs, pensions.


Monthly Household Expenses

We will review these in person, but you can download and complete the Monthly Expenses Worksheet.

Credit Report

We can obtain this during your visit, or you can obtain a copy for free at www.annualcreditreport.com or by phone at (877) 322-8228.

Evidence of Insurance

Coverage declaration sheet(s) for any motor vehicle, homeowners or life insurance policy that you own.

Domestic Support Obligations

If you are paying any child support or alimony, a copy of the separation agreement and/or court order showing the amount of the domestic support to be paid.

Trust Documents

If you have created a trust or are the trustee or beneficiary of a trust, a copy of the Declaration of Trust.

Collections Paperwork

Including correspondence from collection agencies, documentation of any lawsuits brought against you and judgments issued against you.

Mortgage Statement(s)

If you own your home and have a mortgage, a copy of a recent statement for each mortgage.
